New Families to Our Programs
If you are new to our program, please fill out the Faith Formation Registration form (and if necessary, Request for Reception of Sacraments). You may either:- Return signed form(s) to parish office with payment.

If your family is returning to our program, please fill out the Returning Families Registration form (and if necessary, Request for Reception of Sacraments). You may either:- Return signed form(s) to parish office with payment.
- Scan and email signed form(s) to [email protected] and pay online below.
Sacramental Preparation
If you have a child who will be entering our Sacramental Preparation program and has not been baptized in one of the churches in our parish, please be sure to provide a sealed copy of their sacramental certificates at the time of registration.
